| Each capsule contains: Mycophenolate Mofetil….. 250mg | PHARMACOLOGICAL CLASSIFICATION Immunosuppressant MYCOCEPT is rapidly absorbed following oral administration and hydrolyzed to form MPA, which is the active metabolite. MPA is a potent, selective, uncompetitive, and reversible inhibitor of inosine monophosphate dehydrogenase (IMPDH), and therefore inhibits the de novo pathway of guanosine nucleotide synthesis without incorporation into DNA. Because T- and B-lymphocytesare critically dependent for their proliferation on de novo synthesis of purines, whereas other cell types can utilize salvage pathways, MPA has potent cytostatic effects on lymphocytes. MPA inhibits proliferative responses of T- and B-lymphocytes to both mitogenic and allospecific stimulation. Addition of guanosine or deoxyguanosine reverses the cytostatic effects of MPA on lymphocytes. MPA also suppresses antibody formation by B-lymphocytes. MPA prevents the glycosylation of lymphocyte and monocyte glycoproteins that are involved in intercellular adhesion to endothelial cells and may inhibit recruitment of leukocytes into sites of inflammation and graft rejection. Mycophenolate mofetil did not inhibit early events in the activation of human peripheral blood mononuclear cells, such as the production of interleukin-1 (IL-1) and interleukin-2 (IL-2), but did block the coupling of these events to DNA synthesis and proliferation. PHARMACOKINETICS MYCOCEPT is rapidly and extensively absorbed from the gastrointestinal tract, and undergoes presystemic metabolism to active mycophenolic acid. It undergoes enterohepatic recirculation. Mycophenolic acid is metabolised by glucuronidation and excreted primarily in the urine; about of a dose is recovered in faeces. Mycophenolic acid is 97% bound to plasma albumin. The half-life of mycophenolic acid after oral administration of Mycophenolate is about 18 hours. INDICATIONS MYCOCEPT is indicated for the treatment of · Renal Transplantation · Cardiac Transplantation· Hepatic Transplantation | BEFORE YOU TAKE MYCOCEPT Do not take MYCOCEPT Tablets if you have:If you are allergic (hypersensitive) to mycophenolate mofetil, mycophenolic acid or any of the other ingredients of Mycocept. - if you are pregnant, planning to become pregnant, or are breast-feeding
If you have stomach or intestinal problems, including ulcers, kidney problems, or a rare hereditary deficiency of hypoxanthine guanine phosphoribosyl-transferase (HGPRT) such as Lesch-Nyhan syndrome or Kelley-Seegmiller syndrome. HOW TO TAKE MYCOCEPT You must take your medicine as the doctor tells you. It is important to take MYCOCEPT Tablets at the right time. Your doctor will decide which is the best dose for you, depending on your illness. Kidney TransplantAdults:The first dose will be given within 72 hours after the transplant operation. The recommended daily dose is 4 tablets (2 g of the active ingredient) taken as 2 separate doses. This means taking 2 tablets in the morning then 2 tablets in the evening.Children (aged 2 to 18 years):The dose given will vary depending on the size of the child. Your doctor will decide the most appropriate dose based on body surface area (height and weight). The recommended dose is 600 mg/m2 taken twice a day. Heart TransplantAdults:The first dose will be given within 5 days following the transplant operation. The recommended daily dose is 6 tablets (3 g of the active ingredient) taken as 2 separate doses. This means taking 3 tablets in the morning then 3 tablets in the evening.Children:No data are available to recommend the use of MYCOCEPT in children who have received a heart transplant. Liver TransplantAdults:The first dose of oral MYCOCEPT will be given to you at least 4 days after the transplant operation and when you are able to swallow oral medications. The recommended daily dose is 6 tablets (3 g of the active ingredient) taken as 2 separate doses. This means taking 3 tablets in the morning and then 3 tablets in the evening.Children:No data are available to recommend the use of MYCOCEPT in children who have received a liver transplant. IF YOU MISS A DOSETake it as soon as you remember. If it is nearly time for the next dose, do not take the one you missed. Take the next dose at the usual time. Do not take a double dose to make up for the dose that you missed. If you miss more than one dose, or you are not sure what to do, ask your doctor or pharmacist. WHAT TO DO IF AN OVERDOSE IS TAKEN If you accidentally take too many capsules, contact your doctor or nearest hospital accident department immediately. TAKING OTHER MEDICINEPlease tell your doctor if you are taking or have recently taken any other medicines, including medicines obtained without prescription, your doctor may wish to alter your dose o MYCOCEPT f if you are taking any of the following:· Azathioprine or other immunosuppressive agents· Cholestyramine · Rifampicin · Antacids· Phosphate binders· Vaccination SIDE EFFECTS Like all medicines, MYCOCEPT can cause side effects in some people.The most common side effects are upset stomach, nausea, vomiting or diarrhea, gas, tremors, sweating, flushing, insomnia, pain, rash, mood changes.Tell your physician, if side effects include: unusual weakness, unusual or easy bruising, swollen glands, skin growths, chest pain (angina), more frequent or painful urination, swelling of the hands or feet, difficulty breathing, constipation.
