 MECHANISM OF ACTION
D CLOT-2 inhibits the vitamin K-dependent synthesis of biologically active forms of the calcium-dependent clotting factors II, VII, IX and X, as well as the regulatory factors protein C, protein S, and protein Z. Warfarin is an antagonist of vitamin K, a necessary element in the synthesis of clotting factors II, VII, IX and X, as well as the naturally occurring endogenous anticoagulant proteins C and S. These factors and proteins are biologically inactive without the carboxylation of certain glutamic acid residues. This carboxylation process requires a reduced vitamin K as a cofactor. Antagonism of vitamin K or a deficiency of this vitamin reduces the rate at which these factors and proteins are produced, thereby creating a state of anticoagulation.

 PHARMACOKINETICS

D CLOT-2 is a racemic mixture of stereo isomers, which are 99 percent bound to albumin. The drug is metabolized in the liver and kidneys, with the subsequent production of inactive metabolites that are excreted in the urine and stool. The half-life of racemic warfarin ranges from 20 to 60 hours. The mean plasma half-life is approximately 40 hours, and the duration of effect is two to five days.10 Thus, the maximum effect of a dose occurs up to 48 hours after administration, and the effect lingers for the next five days.

 INDICATIONS

  

D CLOT-2 is indicated for the treatment of

·          Deep vein thrombosis

·          Pulmonary embolism

·          Transient ischaemic attack (TIA)

·          Blood clots                                              

                     
PATIENT INFORMATION: 
  

  BEFORE YOU TAKE D CLOT-2Make sure you have told your doctor if you:

 ·           Are pregnant, trying for a baby or breast-feeding.

·          Suffer from a blood disorder.

 ·          Have a peptic ulcer. 

·           Have any severe wounds at present.

·           Suffer from high blood pressure.

·           Have had recent surgery, or are due for surgery in the near future.

·          Suffer from problems with your thyroid. 

·           Suffer from liver or kidney problems.

 ·           Consume a lot of cranberry juice.

 

 HOW TO TAKE D CLOT-2

 You must take your medicine as the doctor tells you. It is important to take D CLOT-2 Tablets at the right time. Your doctor will decide which is the best dose for you, depending on your illness. D CLOT-2 comes as a tablet to take by mouth. It is usually taken as Adults 2 to 5 mg/day initially for 2 to 4 days; adjust daily dose according to PT or INR determinations. Usual maintenance dose is 2 to 10 mg/day. 
 IF YOU MISS A DOSE
Take it as soon as you remember. If it is nearly time for the next dose, do not take the one you missed. Take the next dose at the usual time.  Do not take a double dose to make up for the dose that you missed. If you miss more than one dose, or you are not sure what to do, ask your doctor or pharmacist.  

 WHAT TO DO IF AN OVERDOSE IS TAKEN

  If you accidentally take too many tablets, contact your doctor or nearest hospital accident department immediately. 
 TAKING OTHER MEDICINE
 
  • Please tell your doctor if you are taking or have recently taken any other medicines, including medicines obtained without prescription
  • Do not take any aspirin containing products with your warfarin, unless your doctor has prescribed aspirin for you for a specific reason.
 
SIDE EFFECTS

  Less common side effects include:

·          Rashes·          Nausea·          Vomiting·          Diarrhea
 
The most serious side effects of warfarin are bleeding. You must seek medical attention and have an urgent blood test if you experience any of the following side effects:
